In coordination with their departments, Faculty Leaders must also complete a Course Add/Change/Delete Request through the Office of the Registrar. The form is available at: http://iit.edu/registrar/faculty_staff.shtml

This proposal along with the Course Add/Change/Delete Request will be routed for the following approvals, as appropriate:
- Department Chair
- Study Abroad Review Committee
- College/School Dean
- Vice Provost for Academic Affairs

In order to prevent loss of data on those rare occasions when there is a problem with this form, we highly recommend that you save your answers to the questions below in another reliable format and copy them into this form.
