Law students should contact the Chicago-Kent College of Law Registrar's Office for services.

This form is intended for the exclusive use of Illinois Institute of Technology alumni to request their replacement diplomas.

Illinois Institute of Technology will print your name on the replacement diploma(s) as it appears on file with the university. Requests for a name change must be submitted via the Personal Information Change Request form and accompanied by proper documentation as specified in the form.

Replacement diplomas will reflect the current college and authorized signatures.

The cost for each replacement is $55 per diploma, plus shipping costs as noted below. Each replacement includes a digital and physical diploma.
